How Can severe Tightness In The Chest Be Treated?

I ve thought for 2 decades that I had a hiatal hernia. I would get a tightness in the chest that felt like a heart attack (although I never had a heart attack), but it would eventually wouldn t go away. I could stop it immediately if I drank a lot of water. Now I m getting a sore pain in the center of my diaphram if I sleep in a horizontal position. I have to be completely sitting up when I sleep or I get a severe pain that doesn t go away when I drink a lot of water. Can you give advice?

Your symptoms are suggestive of Gastroesophageal Reflux disease (GERD) associated with Hiatal hernia. GERD is due to reflux of stomach acids into the esophagus.
Management of GERD include
-Omeprazole 20 mg once daily
-Avoid spicy, fatty, fried food
-Do not go to bed on a full stomach. Give at least 2 hours between dinner and bedtime.
-Avoid smoking and alcohol
-Avoid carbonated, caffeinated drinks, chocolate
-Avoid medications that upset stomach like aspirin, ibuprofen etc
-Raise the head end of the bed by using wooden blocks underneath bed posts.
-Regular endoscopic surveillance to look for damage to esophageal lining epithelium

If symptoms persist few months after trying the above measures, surgery for hiatal hernia is required.
